We provide patients with physician recommendations from our physicians for medical marijuana.  before you call for an appointment make sure you have the following:

1.  Copy of medical records going back no further than 18 months describing your debilitating condition.  If you do not have these yet we can provide you with the medical release form tso that they can release it to our office.

2.  Recommendation fee of $200.00 or $150.00 if you are a senior, on SSI, Medicare or are disabled.  Payment options are money order, cashier's check or credit/debit card.

3.  Copy of your Michigan driver's license or State ID.

How the process works.

Gather your medical records stating your condition and fax, email or drop off at our office.  Our physician will review your medical records and we will give you a call within 3 business days to discuss your results.  Upon approval we will schedule a time for you to pick up your recommendation and to drop off your payment.  If you have any concerns please call the office at 586 457 6989.  You can fax your information to 888 304 1597.  Please call before you fax your information.
